New Publications

1.       Broad-range 16S rDNA PCR followed by sequencing: a meaningful method for microbiological investigation of heart valves (Tkadlec et al. 2018)

At this year’s ECCMID, Tkadlec and his colleagues presented a study in which they assessed the role of PCR in microbiological examination of heart tissues. 68 heart valve samples from 61 patients with infective endocarditis (IE) have been investigated. Each sample was examined by culture as well as by broad-range 16S rDNA panbacterial/18S rDNA panfungal PCR. Culture and PCR positive results were evident in eight samples and additional 43 samples were found positive by broad-range 16S PCR only. Sensitivity of culture was 16.7% (8/48), while sensitivity of PCR was 100%. Specificity of culture and PCR were 100% and 85% (3 false PCR positives detected), respectively. 16S rDNA PCR enabled detection of a broad range of microorganisms including the fastidious ones B. quintana and A. defectiva.

2.       Broad-range 16S/18S PCR and sequencing diagnosis of infectious endocarditis using automated pathogen enrichment and DNA extraction (Rubalskii et al. 2018)

On the occasion of this year’s Infectious Diseases Diagnostics Conference, Rubalskii and his team presented results obtained with the SelectNA™plus/Micro-Dx™ diagnosis of 97 samples from 52 patients under suspect of infectious endocarditis (IE). Specimens included mainly excised heart valves and blood, but also pacemaker electrodes, haematomas, pericardial abscesses and thrombi. The patient-related positivity by culture as a reference and SelectNA™plus/Micro-Dx™ were 42% (22/52 patients) and 58% (30/52), respectively. The molecular test detected IE-typical pathogens as well as Corynebacterium diphtheriae in 11 culture-negative patients. Moreover, rare potentially infectious agents were identified including Abiotrophia defectiva, Bartonella quintana, Enterobacter cloacae and a fungus, Pseudallescheria boydii.

3.       Infective Endocarditis – Value of 16S/18S broad-range rDNA diagnosis of pathogens (Lustig 2018)

Infective endocarditis (IE) is usually diagnosed by clinical review, echocardiography, vegetation on heart valves and blood culturing. A major limitation is that blood culture stays negative in up to 31% of cases and is limited in the detection of rare pathogens and mixed infections. Culture-negative IE may cause delayed or inappropriate antibiotic treatment with potentially negative clinical outcome. Broad-range PCR targeting bacterial and fungal rRNA genes together with amplicon sequencing is an evaluated and widely used method of IE pathogen diagnosis. In this application note clinical results of latest studies are discussed that involved protocols of Molzym’s DNA-free reagents and DNA-extraction kits for the direct broad-range PCR and sequencing diagnosis of heart valves for bacterial and fungal pathogens.
